ALL PARTICIPANTS MUST LIVE IN THE OCOEE HIGH SCHOOL BOUNDARIES
AND MUST BE BETWEEN AGES 5-15 AS OF JULY 31, 2008
Important Notice: Since Pop Warner only allows 22 players on the Tiny Mite team, numerous children have been turned away. In an effort to allow more participation for players ages 5-7, we are bringing back the flag football program. All 5 year olds will play flag and the overflow of 6-7 year olds will have the choice to play flag football instead of sitting out a year. More details will be provided at registration. The success of this program, however, relies solely on player and parent participation.
REGISTRATION REQUIREMENTS: NO EXCEPTIONS!!
(without ALL of these items, your child will NOT be able to register)
RETURNING PLAYERS – we are not able to use paperwork from years past. Please bring ALL required documents. Again, NO EXCEPTIONS!!!
•Copy of child’s birth certificate - must have State seal on it.
(We cannot accept a birth certificate issued by the hospital)
•Current picture of your child, 2” x 2” in size preferred
•Copy of insurance card, if you have insurance
•Registration fees (cash or check) – this includes equipment and uniform
o Tackle - $125
o Flag - $65
•Parent and child must be present to register. Players will be trying on uniform pants,
so make sure they wear under garments.
Other Important Notes:
•A current physical is required. It must be on the Pop Warner Medical History form (see website) and dated after January 1, 2008. Dr. Jo Reeves does Pop Warner physicals for $10 cash. You must call for an appointment 407-656-0390.
•All participants MUST turn in a copy of their last report card from this current school year (2007-2008), showing all four (4) semesters prior to August 1st. You can mail it to: Scholastic Coordinator, PO Box 748, Ocoee FL 34761 or turn it in at equipment handout (date – to be determined).
•The Tiny Mite (ages 6-7) team is only allowed 22 players; all other teams are allowed 35 players, per Pop Warner. This means once a team is full, there will be no more players accepted on that team.
