Registration Requirements - All Participants must:
•Live in the Ocoee High School District
•Be between the ages of 5-15 as of July 31, 2008
•Provide a copy of birth certificate - must have State Seal on it.
We cannot accept a hospital issued birth certificate
•Provide a CURRENT photo (2”x 2”)
•Provide a copy of insurance card (only required if you have insurance)
•Registration Fee $75 - (check or cash)
•Parent or Guardian MUST be present to register
Other Important Notes
•A current physical is required prior to August 1st. The Pop Warner Medical History form (available below as a PDF file) is the ONLY FORM ACCEPTED and it must be dated after January 1, 2008. COPIES ARE NOT ACCEPTABLE.
•Physicals can be scheduled with Dr. Jo Reeves at 407-656-0390 for $10 cash.
•ALL participants MUST turn in copy of their last report card from the current school year (2007-2008). It must show final grades for all FOUR (4) semesters. This can be turned into the head coach or mailed to:
Scholastic Coordinator, P.O. Box 748, Ocoee, FL 34761 by June 30, 2008
PLEASE THIS IS VERY IMPORTANT WE HAVE IT BY THAT DATE!
Please - make a copy of your report card and physical forms for your records, as we cannot return these items to you any time during or after the season.
Only 35 girls will be aloud on each team, this is a Mid Florida rule!
There will be no exceptions to this!
